Our story
Aponot was started by a team that met at MIT and spent years as operators in the home-care industry. We’ve run high-performing home-care agencies, so we know how to deliver industry-leading care, support caregivers through a demanding role, and navigate the complex systems — Medicaid, long-term-care insurance, managed care — that surround it.
Running those agencies, we saw the same thing over and over: the daily, in-home work our caregivers did quietly prevented expensive acute events — the fall caught early, the medication error spotted, the decline flagged before it became an ambulance ride. But nobody paid for that value. Non-skilled home care sat outside value-based care entirely, billed by the hour, disconnected from the outcomes it produced.
We built Aponot to close that gap: to aggregate home-care agencies into a single network, overlay what a decade of value-based care has already proven, and connect the home to the payers who carry the cost of avoidable care — so the value our caregivers create every day finally shows up where it should.
We build from inside the work, not above it. Aponot is built on CareUp, our own licensed Florida home-care agency — so every protocol and product decision starts from how care actually gets delivered.
